Who We Are
Community Care Solutions (CCS) is an Alabama non-profit formed by the SARCOA Area Agency on Aging for the purpose of contracting care management services to health care entities through a network of community-based organizations.
SARCOA, as the parent of CCS, provides the operational capacity for administering contracts on a regional or statewide basis. SARCOA has a 35-year history in the care management business, and has the quality, technology, compliance, and business experience required to operate on a statewide or regional basis.
Our Partners
CCS partners with knowledgeable community-based organizations, like Area Agencies on Aging, who have decades of experience in social determinants of health and case management. Community-based organizations offer broader geographic coverage and an understanding of area resources and culture.

Financial Model
CCS participates in various payment models including fee for service, program-based, per referral, pmpm, and risk-based. Our provider partners benefit from the option to elect risk or non-risk-sharing contracts, when available.
